Beth Israel Lahey Health – Home Infusion Services is seeking a Patient Liaison who will be the face of the program across system hospitals. You will build clinical relationships with patients and families, introduce home infusion services, identify discharge barriers, and explain medication self‑administration.
You will collaborate with nurses, pharmacists, case managers, and discharge planners to support smooth transitions home, review insurance questions, and educate patients on infusion
