Key Responsibilities:
• Conduct psycho-social assessments within 24 hours
• Develop and coordinate care plans with the healthcare team
• Offer short-term counseling and behavioral health support
• Advocate for clients facing social determinants of health
• Conduct mental health and suicide risk assessments
Requirements:
• Master’s Degree in Social Work from an accredited program
• Minimum two years of clinical experience
• Current registration with British Columbia College of Social Workers
• Travel capability between different UPCC sites
• Effective teamwork and independent working skills
