In this dual role, you will assess, plan, and provide care for patients with complex health needs. Collaboration with healthcare professionals will be key to successful interventions and patient education. You'll have the chance to significantly influence rehabilitation efforts and community reintegration processes.
Key Responsibilities:
• Conduct thorough health assessments for patient evaluation
• Communicate effectively with inter-professional teams
• Monitor and modify treatment plans as needed
• Provide both crisis and routine nursing care
• Contribute to development of health programs
Requirements:
• Active RN status with the College of Nurses of Ontario
• Full scope experience with unpredictable health outcomes
• Skill in crisis intervention and comprehensive assessment
• Knowledge of relevant nursing legislation
• Excellent communication and teamwork abilities
