Support individuals in overcoming substance use and mental health issues as an Intensive Case Management Worker at Island Health. This position involves community outreach efforts across various sites.

In this vital role, you will work with a diverse population to provide essential life skills training and emotional support. As part of a dedicated outreach team, your efforts will help individuals navigate complex care systems while promoting empowerment and stability. Your work will align with core values of health equity and community collaboration.

Key Responsibilities:
• Deliver outreach services to individuals with addiction issues
• Implement individual care plans with multidisciplinary input




• Lead life skills workshops and educational sessions
• Report and observe changes in client health
• Facilitate positive client interactions and relationships

Requirements:
• Certification in addiction counseling or social services diploma
• At least two years of relevant experience
• Valid Class 5 BC Driver's License
• First Aid and CPR certification
• Robust crisis intervention skills
