In this full time position, you'll play a key role in assessing and supporting clients while addressing health barriers in the Waterloo Region. Utilize your nursing skills for direct primary care, including coordination of health education and community outreach programs. You'll collaborate with an interdisciplinary team committed to improving health outcomes for diverse populations.

Key Responsibilities:
• Conduct comprehensive client history and prepare for exams
• Triage patient needs effectively and compassionately




• Screen minor episodic illnesses and provide appropriate care
• Assist with clinical functions and manage patient flow
• Contribute to health promotion programs and evaluations

Requirements:
• Active registration as a Registered Practical Nurse
• Two or more years of nursing experience, preferably in community care
• Knowledge of community resources for client support
• CPR certification and excellent communication skills
• Ability to work independently and within a team
