In this role, you will manage a case load of 12 clients who require substantial assistance with housing, health appointments, and social programming. Key Responsibilities:



• Manage a case load of 12 complex clients effectively • Provide support for health appointments and housing stabilization • Develop and facilitate social and recreational programming • Assist clients in financial management and personal planning • Collaborate with nurses, support workers, and guest relations staff

Requirements: • Bachelor's Degree in Social Work or a related field • 1-2 years relevant experience in case management • Saskatchewan Class 5 driver's license required • Current First Aid/CPR certification necessary • Ability to become a registered social worker or occupational therapist
