Job Summary

Elevate NWO is seeking a compassionate and organized Linkage to Care Coordinator to join our Community Health Services team.

Positioned at reception, the Linkage to Care Coordinator is often the first face and voice someone encounters when accessing Elevate NWO. The position manages the daily flow of people accessing the Drop-In Centre, clinic, and other services while providing client support, harm reduction, service navigation, and administrative coordination.

The role includes managing calls, intake, referrals, appointments, clinic flow, follow-up, and communication between clients, staff, internal programs, and community partners.

What You’ll Do

- Welcome community members and manage the flow of people accessing the Drop-In Centre, clinic, and other services.
- Manage calls, coordinate arrivals, notify staff, respond to immediate requests, and connect people with the appropriate program or team member.
- Complete intakes and coordinate referrals, appointments, follow-up, warm handoffs, and connections to services.
- Support access to HIV, HCV, and STBBI testing, treatment navigation, harm reduction services, and other health and social supports.
- Provide harm reduction information, safer-use supplies, naloxone, and related education.
- Assist with the coordination of testing, phlebotomy, immunization, and other clinic and community health activities.
- Maintain client resources, supplies, forms, and a clean, organized, and welcoming reception environment.
- Complete accurate and timely documentation, statistics,



and service tracking using OCASE, electronic medical records, and other organizational systems.
- Support communication between clients, clinical staff, case workers, internal programs, and community partners.

Working Environment

This is a client-facing community health position based primarily at reception, where there is regular and often high-volume traffic from people accessing the Drop-In Centre, clinic, and other services. Work may also occur in office, community, partner, and other service settings.

No two days are the same. The successful candidate must be comfortable working with people experiencing substance use, intoxication, mental health concerns, crisis, overdose, trauma, homelessness, poverty, stigma, and other health and social barriers. The role may include responding to urgent client needs, conflict, crisis situations, and active overdose.

Qualifications

- Post-secondary education in Social Services, Health Sciences, Human Services, or a related field, or an equivalent combination of education and relevant experience.
- Experience working with diverse and marginalized populations, with a strong understanding of harm reduction principles.
- Experience with intake, referrals, appointment coordination, reception, administrative support, or client navigation.
- Knowledge of HIV, HCV, STBBIs, substance use, mental health, housing insecurity, poverty, and community resources.
- Strong communication, organizational, documentation, problem-solving, and decision-making skills.




- Ability to manage competing priorities while maintaining confidentiality and professional boundaries.
- Experience using OCASE, electronic medical records, databases, or similar systems is considered an asset.
- Ability to pass a Vulnerable Persons Criminal Reference Check.
- A valid Class G Ontario driver’s licence, clean driver’s abstract, and access to an insured motor vehicle are considered assets and may be required for some assignments.

Pay: $27.04-$31.05 per hour
