Permanent Full-Time 1.0 FTE (37.5 hours/week)
Pay range: From $24.00 per hour to $30.53 per hour
Reporting to the Program Supervisor, Specialist Navigation, the Specialist Navigator coordinates and manages specialist referrals for participating Edmonton O-day’ min Primary Care Network (EOPCN) member clinics.
Working within clinic electronic medical records (EMRs), Specialist Navigators support referrals from initial review through submission, follow-up and appointment confirmation. The role helps ensure referrals are complete, appropriately processed, actively followed and clearly documented, supporting timely access to specialist care and continuity of care for patients.
Specialist Navigators also work closely with physicians and clinic teams, providing guidance on referral pathways, forms, processes and requirements.
KEY RESPONSIBILITIES
- Review referrals for completeness and ensure required forms, information and supporting documentation are included.
- Submit referrals through the appropriate specialist or centralized referral pathway.
- Track and follow referrals through to completion, including following up on outstanding referrals and redirecting declined referrals when required.
- Ensure patients are aware of their scheduled specialist appointments.
- Maintain clear, accurate and timely documentation of all referral activity within the clinic EMR and EOPCN systems.
- Act as a resource to physicians and clinic teams regarding referral pathways, forms, criteria and processes.
- Communicate with clinic teams when additional information or documentation is required.
- Develop team-oriented working relationships with physicians, clinic staff, specialist offices, patients and EOPCN colleagues.
- Participate in team meetings, training, knowledge sharing and quality improvement activities.
- Contribute to consistent referral-management workflows and documentation standards across the Specialist Navigation program.
These responsibilities reflect the program's focus on referral processing, care coordination, clinic communication, data monitoring and consistent referral practices.
QUALIFICATIONS:
- High school diploma and a minimum of two years of related administrative experience within a family practice medical office, Primary Care Network or comparable healthcare environment.
- Medical Office Assistant (MOA) diploma or other relevant post-secondary education is an asset.
- Knowledge of medical terminology.
- Experience using electronic medical records (EMRs); experience with multiple EMR platforms is an asset.
- Previous experience with specialist referrals or referral management is strongly preferred.
- Strong organizational skills and ability to manage multiple tasks and follow-up requirements in a high-volume environment.
- Excellent attention to detail and clear, professional written documentation.
- Strong communication, customer service and problem-solving skills.
- Ability to work independently and collaboratively as part of a team.
- Proficiency with Microsoft Office and comfort learning new electronic systems.
