Job description:

This unique position in north Surrey is a multi-faceted role which takes the knowledge and skills of an LPN, and uses them in community pharmacy setting. We are looking for someone who has strong business acumen and operational skills and can assist the pharmacy managers with some duties:

LPN roles:

- Daily visit to patients
- LPN and HCA scheduling
- New client intake assessment and initial visit
- Patient In Home Assessment every 3-6 months
- Follow up re: Med Assist Compliance
- SBAR to doctors and follow up as needed
- First point of contact for field staff for clinical related issues
- Audits, on the spot audit at client’s home during rounds
- Education/training new staff and ongoing
- Develop best practices for charting/MAR entry
- Marketing and collaboration with other health care professionals
- Complaint management/resolution
- Staff performance review/coaching/mentoring with pharmacy manager
- Conducting client satisfaction survey
- Updating all client information/files
- Creating new client folders for new patients

ABOUT YOU

- at least 1 year experience in community nursing
- Strong leader & knowledgeable with BCCNP best practice




- Organized and qualified
- Excellent communication skills
- Strong administrative background
- Adaptable and flexible
- Highly motivated with a solid work ethic
- Strong customer service and leadership skills
- Ability to work proactively as a member of a dynamic team
- Enthusiasm to contribute positively in the community
- Multilingual is an asset

KEY REQUIREMENTS

- Valid Class 5 driver’s license and own vehicle
- 1+ years experience as an LPN
- Graduated from an accredited program for Practical Nurses

#HP1

- Currently licensed to practice in BC as a Licensed Practical Nurse
- Open availability, including an evening rotation

Pay: $32.00-$34.00 per hour
