Job Title: Housing Follow Up Case Manager

Contract full-time position (35 hours per week)

Contract Start date: ASAP Contract End date: March 31, 2027

Reports to: Supervisor, Housing Follow Up

Who we are looking for:

This project is part of an Initiative to help individuals and households exiting homelessness to transition and stabilize in safe, permanent housing. Guided by Housing First principles, and working within a Harm Reduction framework, the follow up case manager will provide intensive case management supports to 20 individuals with complex needs to maintain housing and provide linkages to community services.

About the role:

- Develop service planning and linkages, goal setting, and supports to stabilize and sustain tenancies
- Build and maintain responsive relationships with landlords to create positive tenancies, troubleshoot and support the resolution of tenancy issues,



providing advocacy and mediation as required
- Follow all project processes and requirements; participate in case transfer meetings.
- Respond to emergency and crisis situations ensuring appropriate conflict resolution, support, debrief and follow up
- Accompany clients to appointments and Landlord and Tenant Board hearings as required
- Maintain accurate case management records and complete statistical and qualitative reporting as required by funder and agency
- Attend and participate in team, agency and City of Toronto meetings and trainings.
- Other duties as required.

About you:

- Post-secondary degree or college diploma in human services
- Proven experience providing case management services to diverse communities
- Demonstrated understanding of housing, health, trauma, substance use, financial, legal, and other issues that impact clients’ lives
- Strong understanding of market rent and subsidized housing systems and policies
- Demonstrated understanding and commitment to harm reduction framework and housing-first approach
- Ability to work independently and as a member of a team
- Ability to manage issues tactfully, and relationships with professionalism, and resolve conflicts
- Knowledge of community resources for homelessness, health, mental health and substance abuse
- Strong organizational, time management,



written and oral communication skills
- Valid G driver’s license and vehicle for daily use for client meetings and appointments
- Valid Vulnerable Sector Check required

Working Conditions

The Follow-Up Case Manager works from various locations in the community, meeting with clients in housing units and in the clients’ buildings, public places and offices. Case Manager also works from an office at 4155 Sheppard Avenue East and attends meetings and appointments across the City of Toronto as required

Hourly Rate: $32.97/hr
