POSITION SUMMARY:

The FASD Program Mentor will be responsible for providing a support system that will assist clients & families, by applying the Home Visitation Model of Care. Mentorship will include the development of a comprehensive service plan that includes support for family dynamics, addictions and overall wellbeing. Duties will include conducting family assessments, developing and implementing short term interventions (including drug & alcohol treatment) through effective case management to stabilize families in crisis and refer to existing community resources for longer term client & family support programs.

Responsibilities include, but will not be limited to:

· Develops comprehensive service plans that meet the needs of the client/family.

· In conjunction with the family and client, identifies and prioritizes client/family goals and needed services that will assist with client/family stabilization (i.e., income support, treatment services, housing, food, clothing, etc.) in the form of a Service Plan.

· Assist the Coordinator in the development and implementation of a quality improvement plan.

· Participates in extensive training as identified by the Coordinator.

· Supports and reinforces the principles, purpose and processes associated with the program.

· Advocate for the individual and/or family to other service areas and organizations in regard to a diagnosis and implications of an FASD diagnosis





· Teaches basic life skills and assist the client/families with parenting and household management.

· Seek reliable housing options to secure a safe environment for the family and children (Includes on and off reserve agencies).

· Ensures client confidentiality in all matters related to diagnosis and file management

· Maintains and strengthens positive working relationships with other service providers

· Provides mentorship and support to professionals and paraprofessionals.

Education & Experience:

· Minimum of a Certification in Social Work, Psychology, Sociology, Addictions or related Human Services field; preferred Diploma.

· One (1) year experience in the addictions field.

· Exceptional computer skills

· Familiarity in working with medical and professional individuals/organizations

Knowledge, skills and abilities:

· Extensive knowledge in working with the First Nations community; knowledge of Blackfoot culture and/or language an asset

· Knowledge of interviewing and information gathering techniques

· Demonstrates a high level of writing ability; able to write reports in a coherent and competent manner

· Ability to provide tactful, courteous and effective service with Siksika Nation clients, agencies, and relevant parties

Job Type: Full-time

Ability to commute/relocate:

- Siksika 146, AB: reliably commute or plan to relocate before starting work (preferred)

Experience:

- Working with indigenous peoples: 3 years (required)
