The Role: Case Manager (Full-time) The Case Manager supports their clients in finding and maintaining housing. With a focus on harm reduction this scattered-site housing program aims to house youth in the community of their choice. Further,
Case Managers aim to reduce barriers for their clients, including those of housing, basic needs, medical appointments, and advocating for system supports, mental health services and addiction counseling.
Case Managers have an assigned caseload for which they manage and support. Ability to have challenging conversations, surrounding case planning, program expectations, etc. Support in re-connecting and building their natural supports Case Management Assist clients in all areas of case management, including areas such as housing, life skills development, cooking, cleaning, employment, education, and budgeting.
Housing supports may include -Navigation of the housing and rental markets; work with the Trellis Housing Liaison for appropriate or affordable housing; expanding understanding of Housing Rights as needed. Support clients in gaining financial independence and building a life around the budget based on the finances of the client. Work with youth and their families, both individually and collectively, to negotiate boundaries, manage conflict, and improve communication all aimed at family reunification.
This might include crisis resolution, mediation, safety planning, advocacy,
facilitated referrals, and system navigation. Navigate referral processes to ensure that clients are able to access Queer safe, Indigenous safe, and any other Cultural Supports as needed. Communicate with community partners in a professional and client centered way.
Assist in goal planning documentation for each client, and the necessary quarterly/ annual updates. Foster a culture of inclusion, diversity, and truth and reconciliation. Participate in supervision meetings, performance reviews, and mandatory trainings as per the Trellis Training Matrix.
Emotional
Intelligence and Regulation Versatile Creative and Problem Solving A Diploma or Degree in a Human Services Field (equivalencies also considered). ~2-5 years of experience working with individuals and/or families with a strengths-based and trauma-informed lens (lived experience considered & an asset). ~ Knowledge of brief therapeutic strategies, housing first, harm reduction, crisis management, and/or Indigenous social work practices an asset. ~ Can work Monday to Friday with flexibility to accommodate families on occasional evenings and weekends. ~ Current Standard First Aid CPR-C, ASIST, and TCI considered an asset. ~ Able to obtain clear Criminal Check with Vulnerable Sector Search and Child Welfare Intervention Check.
