Case Manager – Patterson House Program (Full Time – Permanent) CMHA Toronto’s Patterson House program is seeking a Case Manager.

The Patterson House

Program is delivered in partnership with the City of Toronto and include transitional and permanent housing for individuals who have experienced homelessness. Reporting to the Program Manager, the Case Manager will facilitate the placement and support of Patterson House clients into transitional and supportive housing within a 24/7 staffed CMHA operated building.

Outreach and assessment Skill development and goal planning Providing face-to-face visits to clients in the community Minimum undergraduate degree in Social Work/Social Science or related field; A minimum of 3 years full-time experience in direct service delivery to adults with responsible and complex mental health issues, including those with concurrent disorders.

Knowledge of mental health, the criminal justice system, concurrent disorders, psychosocial rehabilitation, recovery principles and supportive housing strongly preferred. Strong assessment,



service planning and crisis prevention/intervention skills. The successful applicant must have access to a car and insurance for workplace use.
