The Opportunity

As our Care Manager, you will have a significant amount of autonomy and responsibility for the clinical experience of our clients and families.

You will work closely with Vytality's Co-Founders and Co-Presidents, our internal team, caregivers, clients, families and community health-care partners.

This is not a desk-only position.

It is a relationship-driven, community-based leadership role for someone who enjoys variety, can confidently move between clinical care and people leadership, and is comfortable making decisions.

No two days will look exactly the same.

One moment you may be completing an assessment with a new client and their family.

The next you may be helping a family navigate Alberta Health Services, supporting a caregiver through a change in condition, developing a complex care plan, coaching a team member, or helping coordinate care for someone approaching end of life.

What You'll Do

Create exceptional client and family experiences

- Conduct comprehensive client assessments and reassessments.
- Develop highly individualized care plans based on each client's medical, cognitive, functional and personal needs.
- Build trusted relationships with clients and families.
- Act as a key resource when care needs change or situations become more complex.
- Help clients and families understand and navigate available healthcare and community resources.
- Coordinate with other health-care professionals to support continuity of care.
- Help prospective clients and families understand their options and determine whether Vytality is the right fit for their needs.
- Work closely with the HR Manager to manage schedules for clients and caregivers using our matching process.

Lead, coach and support caregivers





Our caregivers are at the heart of the client experience.

You will

- Mentor, coach and support caregivers.
- Provide hands-on education and training.
- Help caregivers understand complex care plans and changing client needs.
- Support caregivers working with clients living with dementia, chronic illness, complex medical conditions or palliative needs.
- Partner with our Scheduling and HR teams to help ensure the right caregivers are matched with the right clients.
- Help develop caregiver confidence and clinical competency.
- Step into difficult situations with calm, clarity and leadership.

Navigate complexity

You will often be the person families and team members turn to when the answer isn't obvious.

We need someone who can:

- Assess a situation quickly.
- Ask the right questions.
- Identify risks.
- Make sound clinical decisions.
- Communicate clearly.
- Find solutions.
- Know when to escalate.
- Follow through.

You should be comfortable working independently while also knowing when collaboration is required.

You can confidently work within Microsoft 365 and learn client-care, scheduling and communication platforms.

Passionate about seniors and families.

You understand that behind every care plan is a person and a family placing enormous trust in us.

Minimum Qualifications

We want to be transparent about what this role requires. Applicants must meet all of the following to be considered:

- Current registration and good standing as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in Alberta.




- Professional experience working with seniors and geriatric clients.
- Demonstrated dementia and Alzheimer's care experience.
- Demonstrated palliative and end-of-life care experience.
- Experience working within Alberta's health-care system, with robust knowledge of health-system navigation and available resources.
- Experience developing, implementing and managing individualized care plans.
- Minimum two years of leadership or managerial experience.
- Minimum three years of local driving experience.
- Valid Alberta Class 5 Driver's Licence.
- Clean driving record.
- Reliable personal vehicle and ability to travel throughout Calgary and surrounding communities.

Experience That Would Make You Stand Out

We would be especially interested in candidates with experience in:

- Home care, case management or community nursing.
- Dementia programs, palliative care or complex care.
- Alberta Health Services or another Alberta health-care environment.
- Client-Directed Home Care.
- Cognitive or functional assessments.
- Caregiver education and clinical training.
- Leading multidisciplinary or caregiving teams.
- Learning new systems including scheduling software, CRMs, and MS Office.

-AlayaCare A Note About Flexibility

Home care happens outside the walls of an office.

This position includes travelling to clients throughout Calgary and surrounding communities and requires flexibility when client or caregiver needs arise.

This role includes overnight on-call, one week in three. Our office supports clients from 6am to 10pm daily, so overnight calls are infrequent. On-call weeks are paid on top of your salary and we provide a monthly phone allowance. You will also step in to provide direct client care or hands-on caregiver training when required.

If you are looking for a predictable role where every day follows the same schedule, this likely won't be the right fit.
