Posted Date: Temporary: Full time Temporary contract until March 31, 2027 with potential to extend Education: University Bachelor's Degree Case Management Community Outreach Response Team (CORT) The goal is to improve health and social outcomes through quick intervention and coordinated case management. This is a temporary contract to March 31, 2027, with potential for renewal. Temporary position until March 31, 2027 with an opportunity to extend.

Support people living with HIV affected by mental illness, addictions, homelessness, poverty, and related barriers. Coordinate psychosocial and medical care plans and connect clients with appropriate services.



Assist clients in addressing barriers such as housing, income, and addictions.

Assist with identification documents and basic legal needs. Transport and accompany clients to medical, legal, and social appointments. Degree in Social Work, Addictions, or related field. Strong knowledge of community and housing resources and advocacy skills.

Ability to build effective relationships with diverse clients and community partners. Understanding of Trauma-Informed Care, Harm Reduction, and HIV-related issues.
