The Clinical Case Manager is an integral part of a busy, dynamic and growing clinical program at Kinark Child and Family Services, Community Based Child and Youth Mental Health program in the Simcoe region. As a member of an interdisciplinary team, the successful candidate will be expected to exercise robust familiarity and knowledge of case management principles and practices and have awareness of community resources and services available within the children’s mental health sector. In addition, the Case Manager will be responsible for providing leadership and supporting the interdisciplinary clinical team in the coordination and provision of client intake, service delivery, discharge, and post-intervention, as required.
The successful candidate will be required to work some evenings per week based on client needs.
The Clinical Case
Manager position is covered by the Ontario Public Service Employees Union on behalf of its local 355. ~ Kinark has introduced a benefit that gives employees in eligible roles the chance to work partially or fully remote if they choose to opt-in for this benefit. This role is assessed as a Roamer position requiring 3 days in the office per week. ~*The hourly pay range for this position is - Level I:$41.34; Complete on-hold calls / waitlist management services to assess risk and conduct safety planning, when needed Support active clients and their therapists with tasks related to case management Participate in the coordination and management of the waitlist by using clinical expertise to determine and prioritize for service based on mental health needs Participate in program area admission process (stage 2 intake) in collaboration with assigned therapist Liaise with child welfare, education,
health and other community stakeholders and partners Identify and facilitate referrals to internal resources (such as Psychiatry, Psychology, therapeutic groups), as well as to external resources (such as services provided by community partners) Bachelor’s Degree in Social Work or other Health-Related discipline is required. ~ A Master’s Degree in Social Work or Psychology is preferred; A Master’s degree in a Health-Related discipline (e.g.
Occupational
Therapy, Nursing) will be considered. ~ Must be registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) as a Social Worker, or the College of Registered Psychotherapists of Ontario (CRPO). ~3 years experience minimum in a mental health setting is preferred. ~ Strong case risk assessment, safety planning, and crisis intervention skills ~ Training and or experience in Cognitive Behaviour Therapy and Dialectical Behaviour Therapy (DBT) will be considered assets. ~ Strong relationship building, engagement, and meeting facilitation skills ~ Experience with providing a broad range of clinical services and demonstrated effective work with a child and adolescent population who present a variety of clinically challenging issues. ~ are considered as assets for this position. ~ Computer competency -proficiency in Excel will be considered an asset for this position. ~ Travel may be required for this position; the candidate would need to have a valid Ontario driver’s license, access to a reliable vehicle and be willing to obtain automobile insurance which includes driving for business purposes with a minimum liability of one million dollars. ~ Candidates Bilingual in English and French will be given special consideration #
