Your role focuses on proactive outreach, support for families, and connecting them with available services. Key Responsibilities:
- Build trusting relationships with community members
- Conduct proactive outreach and remain visible in the community
- Connect individuals to programs and services
- Assist with identifying service gaps and community needs
- Maintain records of outreach and follow-up activities
Requirements:
- Diploma in Social Work, Indigenous Studies, or related field
- Experience in Indigenous outreach or social services
- Valid driver's license and reliable transportation
- Satisfactory Criminal Record Check
- Willingness to obtain First Aid/CPR certification
