Health seeks a Social Worker to enhance quality of life in its 19-bed hospice. This role requires empathy and commitment, providing therapeutic interventions for end-of-life issues, grief, and loss. You'll organize family meetings and advocate for residents' rights while working collaboratively with an interdisciplinary team to ensure comprehensive support. Key Responsibilities:

- Conduct psychosocial assessments for patients and families
- Develop individualized care plans with the care team
- Provide counseling on end-of-life issues and grief
- Facilitate family meetings and support group sessions
- Advocate for resident rights and preferences

Requirements:

- Master’s Degree in Social Work from an accredited institution
- Minimum of two years' experience in hospice care
- Proficient in Point Click Care and Microsoft Office
- Robust interpersonal and communication skills
- Licensed or eligible for licensure in Ontario
