Full-Time, Permanent Shkagamik‑Kwe Health Centre (SKHC) Reports To: Tracy Joly, Director of Primary Care Hours of Work: Monday to Friday, 8:30 a.m. – The Registered Dietitian provides evidence-based nutrition assessment, counselling, education, and support to clients receiving care through Shkagamik‑Kwe Health Centre. Reporting to the Director of Primary Care, the Registered Dietitian works collaboratively within an interdisciplinary health care team to support health promotion, chronic disease management, prevention, and overall wellness. This role emphasizes client‑centred, culturally safe care that respects individual goals, lived experience, and community context, while aligning with best practices and professional standards of dietetic practice.
Provide comprehensive nutrition assessments, individualized care plans, and follow‑up for clients across the lifespan Deliver nutrition counselling related to chronic disease management, preventative care, maternal and child health, and general wellness Collaborate with physicians, nurse practitioners, nurses, and allied health professionals to support integrated primary care Support clients in setting realistic, achievable nutrition goals that align with their health needs and preferences Develop and deliver individual and group‑based nutrition education sessions as required Contribute to health promotion initiatives and community‑based programming Maintain accurate, timely,
and confidential clinical documentation in electronic medical records Participate in interdisciplinary team meetings, case conferences, and care planning discussions Support quality improvement initiatives and data collection as required Adhere to professional standards, privacy legislation, and organizational policies Promote culturally safe and respectful care grounded in relationship‑building and client empowerment Bachelor’s degree in Nutrition or Dietetics Registered and in good standing with the College of Dietitians of Ontario Experience working in primary care, community health, or clinical settings Experience working with diverse populations and complex health needs Excellent communication, counselling, and interpersonal skills Proficiency with electronic medical records and standard office software Clinical and office‑based setting Regular interaction with clients, families, and health care professionals Predictable weekday hours with no evenings or weekends May involve working with clients experiencing complex health and social challenges Why Join Shkagamik‑Kwe Health Centre Shkagamik‑Kwe Health Centre is committed to wholistic, team‑based care that supports physical, emotional, mental, and social well‑being.
