The salary range for this position is CAD $44.85/Hr.
- CAD $56.02/Hr. Come work as a Social Worker with Vancouver Coastal Health (VCH) at Raven Song Community Health Centre in Vancouver, BC! Within the context of a client centered care model and in accordance with established standards of professional practice and the vision and values of the organization, works as part of an interdisciplinary community team to provide clinical and consultative social work services to individuals with chronic health and mental health issues, poverty and victims of violence. Performs professional social work services such as conducting comprehensive social and psychosocial assessments, preparing comprehensive case histories, providing psycho-education and interventions, establishing, maintaining and enhancing therapeutic relationships, evaluating clients’ response to treatment plan and coordinating individualized goal-oriented treatment. Develops and enhances community communications/relationships, provides consultation services, engages in on-going team planning, development, evaluation and quality improvement activities to achieve excellence in client care. May act as a Designated Responder under the Adult Guardianship Act. Current full registration with the British Columbia College of Social Workers. Master’s degree in Social Work from an accredited university. Two (2) years’ recent related experience working with individuals with mental health issues in a health care setting, or an equivalent combination of education, training and experience. Valid BC Driver’s License and access to a personal vehicle for business-related purposes. Comprehensive knowledge and demonstrated ability to apply case management principles. Broad knowledge of case management and practice within a client and family centred care model for clients with complex health care needs.
Broad knowledge of mental illness issues and care. Broad knowledge of legislation and government policies and procedures that relate to the client. Demonstrated ability to apply systems theory to practice in Social Work assessment and intervention. Demonstrated ability to conduct social and psychosocial assessments. Demonstrated ability to communicate effectively, both orally and in writing, with clients and their families, colleagues, physicians, and other health care staff. Skilled in the use of equipment, and in the techniques appropriate for treatment. Ability to operate related equipment including applicable software applications. Applicants with a Bachelor of Social Work (BSW) degree from an accredited School of Social Work, and a demonstrated equivalent combination of education, training and experience may be considered. As per Ministry of Health policy, all health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks.
