Nous sommes à la recherche de deux gestionnaires de cas bilingues sur le terrain pour soutenir les patients, les professionnels de la santé et les cliniques partout dans les provinces de l’Atlantique ainsi que dans la région d’Ottawa et du Sud-Ouest de l’Ontario dans le cadre d’un programme de soutien aux patients (PSP) en pleine croissance. À titre de gestionnaire de cas sur le terrain, vous serez un point de contact essentiel pour les patients, les médecins, le personnel clinique et les autres intervenants du secteur de la santé tout au long du parcours thérapeutique. Agir à titre de principal point de contact du programme pour les médecins, le personnel clinique et les patients inscrits au programme de soutien aux patients. Collaborer avec les médecins, les cliniques, les pharmacies et les partenaires internes afin de coordonner les soins et de résoudre les barrières à l’accès au traitement.

Établir et entretenir de solides relations avec les professionnels de la santé et les équipes cliniques du territoire. Fournir aux médecins et au personnel clinique des mises à jour concernant le progrès des patients et les activités du programme. Participer aux initiatives du programme et aux activités d’amélioration continue.

Maîtrise du français et de l’anglais à l’oral comme à l’écrit. Formation postsecondaire ou combinaison équivalente de scolarité et d’expérience conforme au cadre professionnel P1. Excellentes habiletés en communication, en organisation et en gestion des relations.

Maîtrise de Microsoft Office et des systèmes de documentation numérique. Capacité à maintenir un espace de travail à domicile sécuritaire permettant la tenue de conversations confidentielles avec les patients. Titre professionnel en soins infirmiers (IAA, IAAA, IA ou équivalent).

Expérience de soutien aux programmes pharmaceutiques spécialisés, biologiques ou d'accès à la thérapie.



Connaissance des médicaments biologiques et des exigences préalables au traitement, y compris les analyses de laboratoire, l’imagerie diagnostique et les protocoles de dépistage. Expérience de construction de relations avec les médecins, les cliniques, les pharmacies spécialisées et les autres professionnels de la santé.

Expérience en service à la clientèle, soins de santé, pharma ou soutien aux patients. Poste sur le terrain offrant du soutien aux cliniques et aux professionnels de la santé dans les provinces de l’Atlantique ainsi que dans la région d’Ottawa et du Sud-Ouest de l’Ontario. Travail à distance ou à domicile lorsque les déplacements ne sont pas requis.

We are hiring two Bilingual Field Case Managers to support patients, healthcare professionals, and clinics across Atlantic Canada and Ottawa/Southwestern Ontario through a growing Patient Support Program (PSP). As a Field Case Manager, you will serve as a key point of contact for patients, physicians, clinic staff, and other healthcare stakeholders throughout the treatment journey.

You will support patient onboarding, facilitate access to specialty therapies, provide education and adherence support, and help ensure an exceptional experience for healthcare providers and patients. This is a field-based role requiring regular travel within your assigned territory and offers the opportunity to play an important role in helping patients access and remain on therapy while building strong partnerships within the healthcare community. Serve as the primary program contact for physicians, clinic staff, and patients enrolled in the Patient Support Program.

Support patient onboarding and program enrollment activities. Support treatment adherence and ongoing patient engagement. Collaborate with physicians, clinics, pharmacies, and internal stakeholders to coordinate care and resolve barriers to treatment.

Build and maintain strong relationships with healthcare providers and clinic teams throughout the territory.



Provide updates to physicians and clinic staff regarding patient progress and program activities. Participate in program initiatives and continuous improvement activities.

Fluency in French and English (written and verbal). Previous experience working within a Patient Support Program (PSP). Robust communication, organization and relationship-management skills. Proficiency with Microsoft Office and digital documentation systems.

Ability to work independently while managing multiple priorities. Valid driver's license and ability to travel regularly throughout the assigned territory. Ability to maintain a secure home office environment suitable for confidential patient conversations.

Experience supporting specialty pharmaceutical, biologic, or patient access programs. Knowledge of reimbursement navigation, patient access, and specialty medication support.

Experience delivering patient education and adherence support. Knowledge of biologic medications and pre‑treatment requirements, including laboratory testing, diagnostic imaging and screening protocols.

Experience building relationships with physicians, clinics, specialty pharmacies and healthcare providers. Customer service, healthcare, pharmaceutical or patient support experience. Travel, Work Environment &

Physical Requirements Field-based position supporting clinics and healthcare providers across Atlantic Canada and Ottawa/Southwestern Ontario. Regular regional travel required; use of a personal vehicle may be necessary. Remote/home-based work when not traveling within the territory. Must have a dedicated workspace that supports confidential patient and healthcare provider conversations. Work may involve managing competing priorities and time‑sensitive patient needs. This is determined by several factors, including performance, experience and skills, equity, regular job market evaluations and geographical markets. In addition to base pay, other compensation, such as an annual bonus or long‑term incentive opportunities, may be offered.
