The North End Community Health Centre in Halifax is seeking a dedicated Case Manager for its Housing First program. The role focuses on supporting individuals to exit homelessness by creating personalized plans and providing ongoing caseload management. You will collaborate with internal teams and community partners, conduct tenant meetings, facilitate housing searches, coordinate move-in readiness, and maintain confidential documentation in line with Housing First principles.
