In this role, you will manage a case load of 12 clients who require substantial assistance with housing, health appointments, and social programming. Your crisis management skills, along with a strong foundation in harm reduction and person-centered care, will be essential in making a positive impact. Key Responsibilities:

- Manage a case load of 12 complex clients effectively
- Provide support for health appointments and housing stabilization




- Develop and facilitate social and recreational programming
- Assist clients in financial management and personal planning
- Collaborate with nurses, support workers, and guest relations staff

Requirements:

- Bachelor's Degree in Social Work or a related field
- 1-2 years relevant experience in case management
- Saskatchewan Class 5 driver's license required
- Current First Aid/CPR certification necessary
- Ability to become a registered social worker or occupational therapist
