Solutions (Hamilton) serves the greater Hamilton and Burlington areas, supporting clients to remain in the comfort of their own home through customized care plans.

Shift Schedule: Monday through Friday, Alternating Weekend Rotation: 7:00am–3:00pm; On‑Call Rotation: 11:00pm–7:00am.

Statutory Holiday Rotation: Provided. As a Visiting Nurse, you will deliver personalized high‑quality care directly in clients’ homes. You will act as a primary care planner, provide holistic care, and enhance your skills through training, mentorship, and preceptorship programs.

Participate in on‑call rotations as required (typically twice per month, overnights) Conduct in‑home assessments and deliver nursing care based on individualized care plans Administer medications, wound care, IV therapy, palliative, pediatric care, and other clinical interventions Monitor patient progress and update care plans accordingly Educate patients and caregivers on disease management, medication adherence, and health promotion Collaborate with interdisciplinary teams and delegate tasks to ensure continuity of care Maintain accurate and timely documentation using electronic health records Paid Time Off: Sick days, training, and education days, including new employee orientation Top Industry Wages: Earn top wages plus hourly wages, premiums,
