Dignity Health Mercy San Juan Medical Center is a 370-bed not-for-profit Level 2 Trauma Center located in Carmichael, California, serving the areas of north Sacramento County and south Placer County for more than 50 years. Responsibilities The incumbent provides care, treatment, and service to Home Health and Hospice patients and their families in their place of residence within their scope of practice. Functions as a member of the interdisciplinary team.

Qualifications

Experience:

1. Two (2) years LVN Experience within the last 5 years
2. Recent experience as an LVN in a Home Health or Hospice Agency preferred.

Licensure:

1. Currently licensed as a LVN in California
2. Current California driver's license, possession of automobile insurance & access to an operating motor vehicle.
3. Current CPR/BLS card approved through the American Heart Association.
