Job DescriptionThe RPN/LPN will enhance the effective management and quality of nursing practice daily in keeping with the standards and expectations set out by governing bodies and internal policies and standards of care.Union Name:Union Local:Union Classification (Title): RPN or LPN as applicableSalary: RPN Visit RateNursing Schedule: As per collective agreementAccess to reliable vehicle in community home care is requiredFor both Non- Union and Union jobs, the below description will apply:General Responsibilities1) Ensure care is provided with established nursing standards and principles, physicians’ orders and administrative policies, as well as, observe, report and record vital signs, symptoms and conditions of residents / patients. Assure observance of required techniques in nursing care2) Assist in assessing patients/residents and develop, implement, review and evaluate care plans. Monitor health, notes changes in condition and assess need for referral to doctors; processes and implement physicians’ ordersQualifications1) Appropriate current certificate of competence from the Approved Institutions for the province2) Registration or eligible for registration with the Approved Institutions for the province3) Previous managerial experience in long-term care or community homecare preferred4) Experience dealing with the cognitively impaired5) Comprehensive knowledge of nursing and health care practices, as well as Knowledge in Infection Control Practices6) Demonstrated commitment to working with seniors7) Ability to present information clearly and effectively both verbally whether in person or over the phone, and in writing; an active listener who projects a friendly demeanour8)
