: Reporting to the Primary Health Care Manager, the Primary Health Care Nurse is an integral member of the primary health care team, providing and coordinating initial, continuing and comprehensive nursing services to patients in the PHC sites.

Nurse promotes quality health care outcomes working within established standards for case management practice.
The Primary Health Care
Nurse initiates and/or participates in community development initiatives and participates in the evaluation of Primary Health Care initiatives within the site.
Human Resources Exemption: No
Experience
- 2 years nursing experience in a community based health care setting preferred
Education
- Bachelor of Science in Nursing (BScN)
Licenses
- Valid Class 5 driver's license
Other Education and Training
- Nursing diploma is acceptable
Knowledge and Abilities
- Conflict resolution skills to effectively handle difficult issues
- Knowledge in the principles of Population Health Promotion
- Knowledge and application of quality improvement principles, tools, and outcomes
- Able to plan/organize/prioritize work w/in a continuously changing workplace
- Knowledge of cross cultural issues and cultural sensitivity
- Strong communication/listening and relationship management skills
- Ability to initiate and exercise critical thinking and decision-making
- Demonstrated understanding of the prevention and management of chronic diseases
- Demonstrates empathy, trust and respect in the provision of care
- Knowledge of group dynamics and facilitation skills.
- Ability to adapt to and implement change

- Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)
- Scheduled hours of work may be reduced when a statutory holiday is recognized
