Become a Nurse Practitioner at Durham Community Health Centre, focusing on addiction and homelessness in a community setting. This part time role provides essential support to vulnerable populations. As part of DCHC's dedicated team, you will deliver comprehensive health services tailored to individuals experiencing significant challenges, including substance use disorders and homelessness.

This role requires your clinical expertise, compassion, and commitment to evidence-based practices, ensuring that all care is holistic and integrative. Your leadership will help promote health and well-being within priority populations in the Durham Region. Key Responsibilities:

- Provide comprehensive, client-centered assessments
- Collaborate in an interprofessional healthcare model




- Conduct intake assessments and document findings
- Support clients in navigating health and community services
- Advocate for health equity and practice safety

Requirements:

- Master's Degree in Nursing with NP-PHC certification
- At least three years of primary care nursing experience
- Knowledge of local health services preferred
- Valid Ontario Driver’s license and vehicle access required
- Basic CPR certification and police check required
