MissionHires is partnering with a modern transitional care facility in West Bedford, Nova Scotia to hire full-time Nurse Practitioners (MRP role). In this on-site, weekday position, you’ll independently assess, diagnose, prescribe, and lead interdisciplinary teams to deliver evidence-based, patient-centered care for adults transitioning from acute care back to home or community. Serve as the Most Responsible Health Care Provider (MRP) for adult patients in a transitional care setting Lead on-site clinical teams and provide direct supervision and clinical expertise Collaborate closely with interdisciplinary teams (restorative care professionals, nurses, allied health) Develop and implement evidence-based, patient- and family-centered care plans focused on safe transition home or to community Uphold regulatory, documentation,
