Nurse Practitioner – Geriatric Primary Care – Residency Opportunity at Fraser Health Authority in Langley, BC. This role focuses on geriatric home-based primary care for frail, home-bound older adults. We are seeking a dedicated Nurse Practitioner (NP) to contribute to Geriatric Home-Based Primary Care.

Collaborate with the Langley Seniors Clinic and other community health services to deliver comprehensive medical care to older adults with complex health needs. Develop and manage individualized care plans focused on chronic disease management, medication optimization, and preventive health. Work within a multidisciplinary team to support safe, effective, and timely care transitions.

Advocate for patients and families, helping them navigate health systems and access appropriate resources. Contribute to quality improvement initiatives and the advancement of best practices in geriatric care.

Residency Program

Clinical Supervision: Dedicated supervision during clinical placements.

Experienced Coaching: Weekly check-ins from an experienced NP during the residency. Be part of our clinical enhancement program with regular in-house skills development such as 12 lead ECG interpretation, Point of Care ultrasound (POCUS), suturing and x-ray interpretation, plus centralized NP orientation. You will be welcomed into a New Graduate NP Community of Practice to support your transition into independent practice.

Maternity

Top-Up: 87% top-up during maternity leave. Current practicing registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM). Recent relevant clinical nursing experience.

Professional/Technical Capabilities Ability to communicate and collaborate with patients, families about health findings, diagnosis, treatment, self-care, and prognosis. Ability to collaborate, consult with and refer patients to physicians and other health professionals when appropriate. Ability to critically assess and evaluate health research literature to determine best practices and implement education and evidence-based research.

Ability to assess population health trends and plan prevention and health promotion strategies. Ability to identify and respond to legal and ethical issues in patient care. Ability to self-assess performance and take responsibility for career development.
