Become an Occupational Therapist with Functional Pathways in the Minneapolis/Saint Paul area focused on enhancing patient functional abilities. Engage in screenings, evaluations, and personalized therapy plans. In this role, you will evaluate residents and develop individual Treatment Plans, ensuring comprehensive rehabilitation services.

Quarterly screenings are essential for identifying residents who require therapy, and you will be responsible for supervising the work of Certified Occupational Therapy Assistants. Reassessments will happen every 30 days to determine the necessity for ongoing interventions. Key Responsibilities:

- Screen residents on a quarterly basis for therapy needs
- Establish individualized Treatment Plans per evaluations




- Perform monthly reassessments for progress monitoring
- Supervise treatment by Certified Occupational Therapy Assistants
- Develop discharge plans for optimal patient independence

Requirements:

- Degree in occupational therapy from an accredited program
- Active state license and certification
- Excellent organizational and communication capabilities
- In-depth knowledge of Functional Pathways policies
- Fulfillment of all certification and screening requirements
