Empower youth as an Outreach Counsellor with the YMCA of Greater Toronto. This full-time role focuses on conducting assessments and delivering case management to assist those facing homelessness. You will engage directly with clients in various settings to evaluate their needs and connect them with crucial services.
This position requires a robust understanding of the challenges faced by marginalized populations and the ability to apply trauma-informed care effectively. Your experience will drive significant change in the community. Key Responsibilities:
- Assess client circumstances in public and institutional spaces
- Develop individualized housing plans and case management strategies
- Connect clients with housing and health services
- Provide brief follow-up to ensure continued support
- Document case management assessments thoroughly
Requirements:
- Degree or diploma in social services or equivalent experience
- At least two years’ experience with homeless or street-involved youth
- Knowledge of housing legislation and support systems
- Valid driver's license and First Aid certification
- Excellent communication and relationship-building skills
