This position will provide outreach support to client(s) referred to Connective by Community Living British Columbia to enable their adjustment to, and success with life in the community.

Casual employeesmust be availablefor at least 14 shifts per month. Shifts available from Monday to Sunday, 0800 - 1600, 0900 -1700, and 1000 - 1800. If you are working full-time, or if you are a full-time student, we ask candidates to have an availability of at least 10 shifts per month (this is subject to employer or enrollment proof).

to provide the assessment that will ascertain needs and postulate intervention possibilities Supports individuals in their housing/residential settings Works with the client to consider issues and creates a mutually agreed intervention and support plan that has measurable short and long-term goals Ensures issues of daily living, including but not limited to housing, nutrition, clothing, hygiene, leisure lifestyle and access, employment and education are addressed in this plan Ensures clients’ ongoing legal, mental, and physical health issues are addressed in this plan Interacts with other professionals, family members, and organizations to ensure services, admissions, and treatments are in the client’s interest and supportive of the long-term goals On the clients’ behalf, is a resource to other professionals, including but not limited to the courts, probation services, and community and institutional psychiatric services Maintains extensive records and case notes in accordance with CLBC policies and standards Functions independently, while remaining a crucial member of the care team Adheres to Connective policies and procedures Is available to work flexible hours as needed, which may include some evenings/weekends, as determined by program/client needs and schedules Is a member of CUPE Local 1936-11 Graduate with a Diploma or Certificate in Social Services and one-year of relevant experience is preferred Experience with clients with acquired brain injuries, a history of conflict with the law,



developmental disabilities and/or spectrum disorders, interaction with social agencies and experiencing psychiatric/personality disorders Extensive knowledge of community services as they relate to persons with psychiatric involvement Experience in assessment, case/care plan development and implementation Excellent written and verbal communication skills in English Ability to successfully complete a Vulnerable Persons Criminal Records Check Personal transportation that is in good repair, insured for business up to $1,000,000 for liability, and which can be used to transport clients Proof of vaccination for communicable diseases may be required as mandated by Government, Health Authority, and/or funder requirements As Connective is a unionized environment, the starting wage is $29.22 per hour.
