Implement prescribed treatment plans under the direction and supervision of a Registered Occupational Therapist and/or Physiotherapist Support clients with exercises, mobility training, strengthening programs, gait training, balance retraining, and functional mobility tasks, as well as other prescribed activities Monitor and document client progress, observations, and responses to treatment (with OT/PT review and oversight) Assist with transfer training, safe lifting techniques, and mobility aid usage Provide caregiver education and support for OT/PT‑directed care strategies Assist with basic equipment set‑up, environmental safety recommendations, and home visit follow‑up tasks Maintain appropriate communication with supervising OT/PT, office staff, and interdisciplinary team members Travel to client homes across the GTA and maintain flexibility as caseload changes MUST have a valid driver’s license and reliable access to a car that can be used for daily work Ability to travel between assignments within designated areas of work; an asset if you can travel to rural/remote areas Must be able to communicate effectively both verbally and in writing in English PSW certification from an approved and recognized educational institution or international nursing/doctored certification Negative TB test completed within the last year Valid CPR/First Aid certificate COVID‑vaccination receipts subject to an exemption pursuant to Ontario's Human Rights Code Immunization for measles, rubella, varicella, tetanus, and diphtheria required; hepatitis B immunization strongly urged Valid criminal record check completed within the last year Ability to identify and report client care concerns to Care Coordinators when needed Current/valid Health and Supportive Care Providers Oversight Authority (HSCPOA) registration is an asset Part‑time availability schedule: Monday‑Friday, with occasional Saturday or Sunday availability as needed. Travel will be required between client homes.
