New or Replacement: New Site: Toronto Western Hospital Department: Pharmacy, Inpatient Unit (patient population may change based on department needs) Reports to: Pharmacy Clinical Site Leader Wage Range: $48.51 - $60.64 per hour.
Shifts: Days, Evenings, Overnights, Weekends The Pharmacist provides comprehensive pharmacy services to patients admitted to Toronto Western Hospital, as a member of the patient care team. Conducting best possible medication history (BPMH) interviews, performing medication reconciliation, review all doctors’ medication orders for appropriateness Identify,
resolve and prevent drug therapy problems with the patient and health care team Follow-up on medication incidents and promote safe medication practices on the unit Facilitate discharge reconciliation Provide direct and/or functional supervision of technician staff Facilitate and support clinical teaching and research activities within the department, while exemplifying UHN’s values of patient needs come first, safety, compassion, teamwork, integrity, and stewardship Possess a Bachelor's of Science Degree in Pharmacy, Doctor of Pharmacy degree or equivalent required Certificate of competency from the Pharmacy Examination Board of Canada required Active registration with the Ontario College of Pharmacists (Part A) required Hospital pharmacy practice experience with patient-centered care experience is a requirement Excellent customer service and interpersonal skills Hospital Pharmacy residency an asset Membership in the Canadian Society of Hospital Pharmacists and/or other professional societies an asset Government organization and a member of the Healthcare of Ontario Pension Plan (HOOPP A flexible work environment Additional perks (multiple corporate discounts including: travel, restaurants, parking, phone plans, auto insurance discounts, in office gyms, etc.) Please be advised that a Criminal Record Check may be required of the successful candidate.
