The Clinical Planner is accountable for onboarding new patients requiring Personal Support Services within an assigned district, through the assessment, implementation, and evaluation of a wide range of clinical and personal care interventions. Perform the initial personal care services for new patients.

Inform patients and families of ParaMed policies and scheduling practices, as well as the expected time of care. Registration in good standing as a RPN/LPN with provincial governing bodies and minimum two years’ experience as a practicing nurse. Working knowledge of applicable legislation including but not limited to Mental Health Act, Substitute Decisions Act, Healthcare Consent Act, Privacy / Personal Health Information Act, Occupational Health and Safety legislation and practices.

Knowledgeable in best practice standards for community health care and services. Demonstrated competency in patient centered care principles and practices. Knowledgeable of community resources available for patients and families.
