Elevate community well-being as a Public Health and Home Care Nurse in Thunder Bay, ON. You will develop and deliver essential in-home nursing services for diverse client needs. This role involves coordinating public health initiatives and providing direct nursing care to HCC clients.

You'll assess individual needs, create care plans, and lead public health education activities, all while collaborating with Community Support Workers and local healthcare teams to strengthen community health initiatives. You play a crucial role in supporting resilience in First Nations communities. Key Responsibilities:

- Plan and implement public health programs in collaboration
- Conduct home visits and develop customized client care plans
- Supervise and provide support to Community PSWs
- Foster relationships with healthcare stakeholders
- Prepare statistical and quarterly reports as needed

Requirements:

- Licensed RN or RPN with Ontario regulatory body
- At least 2 years of relevant nursing experience
- Advanced Foot Care certification an asset
- Excellent written and verbal communication
- Ability to travel to various communities
