Enhance lives as a Registered Practical Nurse with our Mobile Response Team in Brockville, Ontario. Deliver compassionate care for older adults with responsive behaviours related to dementia and mental health challenges. This full-time position at Providence Care involves direct collaboration with RNs and PSWs to provide critical interventions in long-term care settings.

You will assist in transitions and develop tailored care plans, leveraging your expertise in responsive behaviour management. Aiming for person-centred care, you’ll work with a dedicated team to improve the quality of life for clients. Key Responsibilities:

- Provide direct care in collaboration with the MRT RN and PSWs
- Develop assignments and support MRT PSWs
- Collaborate with seniors/geriatric mental health teams




- Utilize evidence-based assessment tools for client evaluation
- Implement Collaborative Care Plans for responsive behaviours

Requirements:

- Current RPN registration with the College of Nurses of Ontario
- RPN Diploma with medication certificate
- Minimum one year related experience with older adults
- Experience in supportive management of responsive behaviours
- Solid written and oral communication skills
