- Complete comprehensive psychosocial assessments for residents
- Provide individual and group counseling for emotional support
- Develop and implement tailored social work plans
- Advocate for residents' rights and access to services
- Coordinate with interdisciplinary teams for optimal care
Requirements:
- Bachelor of Social Work from an accredited program
- Minimum three years in long-term care or related services
- Robust skills in crisis intervention and care planning
- Current registration with the Ontario Social Workers College
- Proficiency in electronic health records
