As a member of the healthcare team, the Social Worker provides direct patient care through the provision of psycho-social screening and assessments; individual and family counseling and treatment; crisis intervention; bereavement counseling; liaison and referral to the community agencies; advocacy and discharge planning.

Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a Social Worker

- Master's Degree in Social Work from an accredited School of Social Work.
- Knowledge of the Healthcare Consent Act, Substitute Decisions Act, Child and Services Act
- Clinical SW experience preferred
- Oncology and counselling experience preferred
- Excellent knowledge of community support and placement services
- Excellent crisis intervention skills
- Demonstrated knowledge and experience in end of life issues
- Awareness and counselling experience with quality of life issues related to treatment, side effect and chronic nature of disease
- Excellent knowledge of community support and placement services
- Proven ability to organize and prioritize patient caseload across several programs
- Proven ability to work as a member of interdisciplinary healthcare teams

- Hospital-based clinical experience in a variety of patient care areas preferred The Hematological Program at the Juravinski Hospital & Cancer Centre provides care to patients with cancer of the lymph glands, blood or borne marrow. Care is provided in a hematology clinic, an oncology day service clinic and an impatient setting by a multidisciplinary team. The primary modes of therapy are intensive chemotherapy, transfusion and antibiotic support for immunosuppression and stem and bone marrow transplantation.
