Make an impact as a Registered Dietitian in the Diabetes Education Program at Taddle Creek. Provide expert patient education and care focusing on diabetes management in a community setting. In this full time role, you will work directly with patients living with diabetes.
Your primary duties include individual counseling and group education, enabling patients to manage their health effectively. You will collaborate with health care providers to create comprehensive care plans, utilizing your deep knowledge of diabetes management and nutrition. Key Responsibilities:
- Manage a caseload of 7-8 patients daily
- Conduct assessments to tailor nutrition plans
- Partner with patients to design their health protocols
- Deliver group education and training on diabetes
- Ensure timely documentation and communication
Requirements:
- Bachelor’s Degree in Nutrition or equivalent
- Registered Dietitian status with the College of Dietitians of Ontario
- At least 2 years of clinical experience
- Certified Diabetes Educator certification required
- Excellent organizational and communication skills
