The Lakeland Centre for Fetal Alcohol Spectrum Disorder (LCFASD) is seeking a Transition Coordinator to guide youth diagnosed with FASD through the shift to adult services. Under the Executive Director, you will develop processes, coordinate referrals, and advocate with families and agencies to ensure seamless transitions.
Responsibilities include building partnerships, maintaining youth service statistics, and promoting confidentiality and ethical practice across transition programs.
