We serve clients across Kawartha Lakes, Durham Region, and Peterborough, supporting individuals with complex needs including dementia, Alzheimer’s, Parkinson’s, palliative care needs, and more.
Position Overview
We are looking for a caring and self-directed Registered Practical Nurse (RPN) to join our team on a casual basis. This role is a blend of community-based home visits, regular shifts at Bob’s Place, and on-call nursing support coverage for our community clients and residential home.
The ideal candidate is confident working independently, comfortable with a variety of clients and care settings, and brings a calm, professional presence — especially in after-hours and on-call situations.
Key ResponsibilitiesCommunity Care (Care Crew Solutions)
· Conduct scheduled nursing visits to clients in their homes across our service areas
· Complete nursing assessments, wound care, medication administration and management, and health monitoring
· Develop, update, and communicate care plans in collaboration with the Clinical Care Manager
· Provide education and support to clients and family caregivers
· Document all visits accurately and in a timely manner in client records
· Identify and report any changes in client condition to the Clinical Care Manager
Residential Care — Bob’s Place (Purple House Services)
· Provide nursing oversight and support during scheduled shifts at our 10-bed care home
· Monitor resident health, administer medications, and respond to any clinical concerns
· Liaise with PSWs and the care team to ensure continuity of care
· Participate in care conferences and update resident care plans as needed
· Ensure documentation is complete and compliant with internal standards
On-Call Coverage
· Available for evening and weekend on-call nursing support for both community clients and Bob’s Place
· Respond to urgent nursing concerns by phone or in person as required
· Provide guidance and clinical decision-making support to PSWs during on-call hours
· Escalate critical concerns per the organization’s escalation protocol
Qualifications
· Current registration in good standing with the College of Nurses of Ontario (CNO) as an RPN
· Experience in community care, home care, or residential/long-term care settings preferred
· Strong clinical assessment and documentation skills
· Experience with dementia care, palliative care, wound care, and medication management is an asset
· Comfortable working independently with minimal on-site supervision
· Excellent communication and judgment — able to make sound decisions in urgent or after-hours situations
· Valid Ontario driver’s license and reliable vehicle (travel between client locations required)
· Current First Aid & CPR certification
· Vulnerable Sector Police Check (required prior to start)
Schedule & Compensation
· Regular shifts: Minimum 1 shift per week (schedule TBD based on operational need)
· Weekend availability: Minimum 1 weekend per month
· On-call availability: Evening and weekend on-call shifts as scheduled — details to be discussed at interview
· Competitive casual hourly rate, commensurate with experience
· Mileage reimbursement for travel between client/home visits
Working Conditions
· Travel required across Kawartha Lakes, Durham Region, and Peterborough service areas
· May encounter clients with complex medical, cognitive, or behavioural needs
· Evening and weekend on-call shifts require availability to respond by phone or in person within a reasonable timeframe
· Collaborative team environment with direct reporting to the Clinical Care Manager
