Overview
Train Alberta is seeking a compassionate and action-oriented Service Navigator to join our Alberta Onsite team. The Service Navigator helps individuals and participants address barriers affecting program participation, employment readiness and job retention. The role coordinates community referrals, arranges approved wraparound supports and follows up to confirm successful connections. This is not a counselling position. It is a hands-on service-coordination role requiring empathy, sound judgement and solid follow-through.

Responsibilities

- Triage individuals who are not eligible for Alberta Onsite and connect them with appropriate community or government services.
- Support accepted participants with barriers affecting program participation, employment readiness or job retention.
- Coordinate referrals related to housing, food security, mental health, addictions, childcare, transportation, financial stability, disability services and other needs.
- Contact service providers, make introductions, assist with appointments and follow up to confirm participants accessed services.
- Arrange approved funded wraparound supports according to participant needs and program guidelines.
- Monitor emerging barriers and adjust support strategies with participants and the program team.




- Participate in case conferences when coordinated support is required.
- Build relationships with Medicine Hat service providers and maintain current referral resources.
- Accurately document referrals, supports, follow-up activities and outcomes in required systems.
- Maintain confidential, organized and audit-ready records.
- Collaborate with the Alberta Onsite team to provide coordinated participant support.
- Assist with facilitation or other program activities when required.
- Follow-up with clients for up to 365 days
- Other duties as assigned

Qualifications

- Post-secondary education in Human Services, Social Work, Community Development or a related field.
- At least two years of experience supporting individuals with complex needs or employment barriers.
- Knowledge of Medicine Hat community services and referral pathways.
- Experience coordinating referrals and helping individuals navigate service systems.
- Strong communication, organization, documentation and follow-up skills.
- Experience with Microsoft 365, CRM or electronic case-management systems.

Job Type: Full-time

Pay: From $26.00 per hour
