The PHCNP reports to the Director of Residential Services and is responsible for the overall health and well‑being of clients in the association's residential care. The PHCNP promotes health protection and quality care delivery and is self‑regulated by the College of Nurses in Ontario. The role involves direct clinical practice, use of a holistic perspective, therapeutic partnership with the client, expert critical thinking, reflective practice, and evidence‑based practice. It also includes expert coaching, consultation, research, leadership, collaboration, and ethical decision‑making.Mandatory RequirementsRegistered with the Ontario College of Nurses at the extended class levelClinical experience in a variety of health care settings (pediatric, women’s health, mental health)Ability to work independentlyValid Province of Ontario ‘G’ Driver’s LicenseCriminal reference check with Broad Record Check reportDuties & ResponsibilitiesPerform comprehensive health assessments, physical and psychosocial assessment of clients and develop and implement a plan of care.Consult with other health care professionals when the client’s condition requires care beyond the scope of practice.
