and transitions between programs.Arrange joint home visits and provide advice on community resources.Deliver health counseling, education, and self‑management support via clinic, telephone, or home visits.Maintain clinical records, including intake screens, assessments, interventions, and progress notes, and track statistical information.Engage in professional development, review literature, attend workshops, and network with health professionals.Participate in department quality improvement, risk management, and research activities.Orient and educate nursing staff and students, acting as a preceptor as appropriate.Contribute to the development and revision of standards, policies, and quality initiatives, providing recommendations to management.Perform other related duties as assigned.QualificationsCurrent practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).One (1) year of recent related clinical experience in assessing and treating complex geriatric and/or adult patients with chronic health conditions in an acute or community/outpatient setting.Valid BC driver’s license and access to a personal vehicle for business‑related purposes.Demonstrated knowledge and competence in gerontology and frailty management.Ability to complete initial and ongoing client assessments and provide therapeutic nursing care.Effective verbal and written communication skills.Ability to integrate and evaluate data from multiple sources to problem‑solve.Knowledge of broad health care services, community resources, and their roles in continuum of care.Sensitivity to diverse cultures and preferences, promoting client‑focused care.Ability to independently manage and prioritize clients with diverse healthcare issues.Teaching skills for health promotion and self‑management.Adaptability to dynamic environments with changing priorities.Teamwork skills within an interprofessional setting.Familiarity with related equipment and software applications.Physical ability to perform the duties of the position.#J-18808-Ljbffr
