Job SummaryReporting to the Client Care Coordinator, the Home Support Worker is part of a health care team that provides assistance to families or persons during times of illness, disability, or inability to properly care for their own needs. The home support worker also supports independence and the ability of the client to remain in their own homes and assists with the activities of daily living.Summary of Essential Job FunctionsProvide personal care such as bathing, toileting, and grooming to the clientComplete housekeeping tasks, meal preparation, and shoppingProvide companionship to the client and accompany them to community events, activities, and appointmentsFollow written plan of care as approved by the Client Care ManagerProvide specialized care such as Alzheimer or palliative care, if requiredDocument and report client progressPromote good mental health for the clientDevelop a caring relationship with the client and the client’s familyProvide emotional and psychological supportEnsure a healthy and secure living and working environment while following agency guidelines on lifting and transfersAdhere to and enforce CareGivers’ Code of ConductReport all Occupational Health and safety hazards and related issues and follows OH&S guidelineMinimum RequirementsMinimum grade 12 high school education and experience as a home support worker ORCertification as a home support worker (Preferred)
