We are seeking a dynamic, community-oriented Dietitian and Diabetic Educator to fill a term vacancy .

Position Overview and Responsibilities Reporting directly to the Community Health Manager and working with the Diabetic and Family Outreach project teams, the Dietitian and Diabetic Educator promotes healthy eating, enhances management and prevention of diabetes through nutritional counseling to diabetic clients, consultation, education and awareness to community members in collaboration with other health professionals.

Clinical support to the Diabetic and Family Outreach Project Team, including providing information on current best practices, program and clinical updates and new resources.

Providing consultation and follow up on case management issues.

Nutrition and diabetic education services to clients,



including providing a comprehensive teaching and monitoring plan for newly diagnosed and/or clients with uncontrolled diabetes with the goal to manage their diabetic regime independently.

Providing teaching to clients, families and caregivers about nutrition, diet and food selection and incorporating appropriate traditional foods into client’s nutritional plan.

Evaluating how clients respond to their diets and check that they are following their diets at home.

Conducting or coordinating group diabetic education on the prevention of and complications of diabetes.

Follow up on referrals made by other health professionals

Providing program and other statistical reports, such as monthly reports, as required in order to facilitate program planning

Qualifications

Member of the Saskatchewan Dietetics Association (SDA)

Willingness to upgrade current skills by attending professional training sessions/workshops as required

An understanding of diabetes disease process, self-management, adult learning and teaching principles

Solid verbal and written communication skills, excellent interpersonal skills

Be self-motivated, creative, willing to learn, eager to help

Must have a valid Class 5 driver’s license and a clean Driver’s Abstract (extensive travel is required)

Current Satisfactory Criminal Record Check and Vulnerable Sector Check

Ability to speak Cree is an asset.
