Duties & ResponsibilitiesPerform duties within the Diabetes Education Program (DEP) teamDevelop and implement diabetes management education programingContribute to the development of policies, procedures, protocols, medical directives and best practice guidelinesDevelop programs for health promotion and disease prevention for diabetic clientsContribute to the development of client centered healing paths, in conjunction with and in collaboration with clients, health care providers and Traditional Healers,
Elders and Medicine PeopleProvide diabetes education and monitoring services to individual clients to better manage diabetesOther related duties as assignedQualifications & ExperienceRegistration with the College of Nurses of OntarioCurrent certification as a Diabetes Educator2 years’ experience in Diabetes Education ProgramingEffective consultation and collaboration skillsAbility to develop program curriculumAbility to function independently and within a multidisciplinary teamRespect for and an understanding of indigenous culture, traditions and teachingsKnowledge of indigenous health issues and the impacts of post-colonial and residential school historiesClinical experience in diabetes managementPay: $38.00-$47.00 per hourWork Location: In person#J-18808-Ljbffr
