Current extended class certification with College of Nurses of Ontario
Experience with chronic disease management and geriatric care
Current vulnerable sector police check

Job Responsibilities:
Responsibility over medical care delivery to LTC residents
Management of secure complex chronic co-morbidities in collaboration with MDs
Collaboration with an interdisciplinary team in the course of care delivery
Provide high quality customer service
Provide clinical / medical education to clients and staff
Participate in care conferencing




Role is 70% clinical and 30% quality improvement, education, and leadership
